- Conducts clinical intensive case management services for eligible veterans and their family members within the HCHV/VASH (Healthcare for Homeless Veterans/VA Supportive Housing) . Provides bio-psychosocial services to chronically homeless veterans and their families or veterans at-risk for homelessness. Collaborates with VA medical center's homeless programs, HUD (Housing Urban Development), local housing authorities, residential/community care programs, and residential rehabilitation programs to ensure that coordination and support is facilitated throughout Veteran's direct care. Interviews veterans for housing and medical support via bio-psychosocial assessments, makes bio-psychosocial diagnoses, and implements/revises a treatment plan accordingly. Creates a therapeutic alliance with each Veteran (and with his/her family) to establish follow-up care, home visits, clinical/community-based referrals, educational services, i.e. Documents progress notes, bio-psychosocial assessments, treatment, referrals, treatment plans and continuum of care via U.S. Department of Veterans Affairs' CPRS (Computerized Patient Record System). Subscribes to the NASW code of ethics. Adheres to regulations and policies concerning confidentiality, computer security, HIPPA regulations, and other sensitive information.
